FORMS AND INFORMATION FOR STUDENT-ATHLETES

In the spring, Penn High School Athletics will host athletic physicals for high school students and 8th graders (incoming freshman) in the Aux gym at Penn High School. The cost is $25 per student. Students and parents should complete the IHSAA physical form prior to attending, with the exception of page 2 (doctor's portion). If you chose to visit your own doctor, you will still need to have your doctor fill out the IHSAA form to take to your family doctor.
Before you can try out, practice or compete, we must have an IHSAA pre-participation physical evaluation form on file!
Please make sure to print the form and have your doctor, or other health practitioner, complete it and return it to the Penn High Athletic Office. To be valid for the Fall sports season, your physical exam must have been completed after the IHSAA designated date in the spring. If you prefer to pick up a copy of the physical exam form, come by the Athletics Office, located next to Door B.
Prior to the second week of Fall 2016 practice, you will also need to complete, sign and return the following:
- Concussion and Sudden Cardiac Arrest Acknowledgement form
- Athletic Accident Insurance ($50.00) – per student per year
- Transportation Fee ($10.00) – per student athlete per sport season
Coaches will have Code of Conduct packets available for their student-athletes, and packets are also available at the Athletics Office. The other required forms can be accessed below.
