Sports physicals offered at Penn on May 19

The Penn High School Athletic Department is pleased to announce that they will once again host the annual Sport's Physicals conducted by doctors from St. Joseph Regional Medical Center. The event takes place on Tuesday, May 19, 2015 in the auxiliary gym at Penn High School from 3:15 p.m. to approximately 5:00 p.m. This is only for high school students and incoming freshmen. The physical covers students to participate in Penn athletics for the 2015-16 school year. Cost is $25.

 

The physical must be completed on this IHSAA Physical Form. The form can also be picked up in the Penn Athletic Office. Athletes and/or parents must complete the form, including all parent/student signatures, prior to attending the physical on May 19.  The doctors will fill out their portion (on page 2) during the physical.
